Dokumenty ICH Q3A, Q3B, Q3C a Q6A veľmi dobre definujú požiadavky na kontrolu a hodnotenie nečistôt a degradačných produktov vo farmaceuticky účinných látkach a vo finálnych liekoch.
Dokument uvádza do praxe koncepciu prahovej hodnoty TTC, ktorá určuje hranicu bezpečného príjmu akejkoľvek genotoxickej nečistoty a dáva ju do súvislosti s akceptovateľným rizikom pravdepodobného vzniku tumorového ochorenia.
Kľúčové slová: hodnotenie bezpečnosti liekov - identifikácia a kvalifikácia nečistôt - genotoxické nečistoty - genotoxicita - riziko-prínos - analytické metódy pre stopovú analýzu - limity akceptovateľného rizika
GENOTOXIC IMPURITIES AND SAFETY OF DRUGS
The documents ICH Q3A, Q3B, Q3C and Q6A very well define the requirements for the control and assessment of impurities and degradation products in pharmaceutically active compounds as well as in final drugs. However, in documents, with the exception of ICH Q3C it is not explicitly specified how the control and assessment of the risk of genotoxic impurities should be performed. Therefore several manufacturers of drugs and drug agencies understood the interpretation of these directions differently. Pragmatic solution of this problem is new document of the European Drug Agency EMEA/SWP about the limits of genotoxic impurities, effective from 1st January 2007. It helps to solve the situations in assessment of safety of drugs when the genotoxic impurity is present and where there is no specific information about its cancerogenic potential. The document puts into practice the concept of threshold value TTC that determines the limit of safety uptake of any genotoxic impurity and connects it with acceptable risk of probable development of malignancy.
Key words: assessment of safety of drugs - identification and qualification of impurities - genotoxic impurities - genotoxicity - risk-benefit - analytical methods for trace analysis - limits of acceptable risk
